To request a special accommodation, please submit a completed Request for Special Accommodations Form and the Student Consent for Release of Records. Ask for the forms to be emailed or faxed to you by NYCDA’s Student Services Associate at studentservices@sft.edu The form, along with appropriate documentation from health care providers, should be returned to Student Services in advance of the start of the school year. Please note that appropriate documentation will be required and reviewed by Student Services and the
Academic Leadership Team, and communicated to faculty, as deemed necessary.
